Payment information

Full Moon Pediatric Therapy operates on a self-pay basis for mobile OT services, with the exception of FAPT/CSA. This model enables us to prioritize delivering personalized OT services in home and community settings, while maintaining balanced caseloads and offering flexibility for sessions without the constraints of insurance.

Our practice is NOT in-network with any insurance providers, including Medicaid or commercial plans. Clients can request an itemized superbill for out-of-network reimbursement purposes. It is important to consult with your insurance carrier to determine specific benefits, as reimbursement is not guaranteed. Superbills are limited to individual occupational therapy services only; group services are excluded.

Payment is due at the time of service or can be purchased in advance through one of our individual or group package options. We use a high-security, online payment processing platform called Square.

Accepted payment methods: Credit/debit cards, HSA/FSA cards, ACH payment, and AfterPay

Full Moon Pediatric Therapy will provide you with a "Good Faith Estimate" explaining the expected costs for occupational therapy services. You have the right to receive a “Good Faith Estimate” explaining how much your therapy services will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.
